5	Letter from the Head of the Department stating that an internal program has been implemented to cover the duties and the letter of consent to cover the duties
6	Written evidence that the loan balances have been settled
7	Whether disciplinary inquiries exist / intend to be held
8	Recommendation of the Head of the relevant Department
9	Confirmation / non-confirmation in the post
10	Certified clear copies of National Identity Card and Passport
11	Signatures and official seals at the designated places of the relevant documents
